Applying for Funding

The Washington State Health Care Authority, through the Community Health Services (CHS) program, awards funding to community clinics for dental, medical, and migrant health care services. These funds are targeted to assist people who are at or below 200% of the federal income guidelines and have no other form of health care coverage such as Medicaid or Basic Health.

Eligibility

Some of the eligibility requirements that must be met to be considered for funding are:

  • Be a private, nonprofit, tax-exempt organization incorporated in Washington State or a public agency under the jurisdiction of a local or county government;
  • Operate as a community health clinic providing primary health care for at least 18 months prior to applying for funding;
  • Have established a sliding-fee schedule for adjustment of charges, based upon the individual's ability to pay;
  • Provide health care regardless of the individual's ability to pay; and
  • Have established policies and procedures reflecting sensitivity to cultural and linguistic differences of individuals served.

General Information

  • The Community Health Services funding cycle is based on the State Fiscal Year (SFY).
  • SFY 2010 covers the period of July 1, 2009 through June 30, 2010.
  • The deadline for submitting all components of your application is 4 p.m. (Pacific Standard Time), Friday, April 3, 2009.

CHS SFY 2010 Grant Application Kit

A complete application consists of the components listed below.

  1. Letter of Transmittal (14.7 KB) - This is a document that you may use as a cover letter to submit with your application.
  2. Agency Information and Signature Page - This is page 3 of Part 1 of the application. We require original signatures. Include with your Letter of Transmittal.
  3. Sliding Fee Scale
  4. Sliding Fee Application (110.2 KB) - This is a document for your reference. We require a copy of the application your clinic uses for sliding fee scale applicants.
  5. Independent Auditors' Report (with Financial Statements and Letters to Management).
  6. Institutional File materials - paper copies only (see instructions for Part 3).
